COMS Interactive Launches Continuum of Care Portals



Managed Care Organizations to More Actively Participate in Entire Care Process



CLEVELAND -- COMS Interactive, LLC (also known as "Clinical Outcomes Management Systems" or "COMS") has announced the launch of a continuum of care portal series.  The new product offering will work in conjunction with the company's disease management product suite, Daylight IQ(TM), and the recently acquired Directed Medical Systems product offerings.  The Directed Medical Systems team are considered subject matter experts in Advance Care Directives and End of Life care solutions.



Daylight IQ(TM), a Software as a Service (SaaS) product, is currently being utilized by individual Skilled Nursing Facilities, large chains and Home Care entities throughout the US.  The intention of this most recent product extension is to more efficiently connect the healthcare continuum by providing information, access and more active participation by the entire clinical team.  The care team includes Hospitals, Managed Care Organizations (MCOs), Accountable Care Organizations (ACOs), Home Care agencies and various Post-Acute providers.
