A.O. Fox Hospital Selects Population Health Management Platform from Phytel

Population health and care management platform will provide higher quality care, increased collaboration between health providers and a better patient experience

DALLAS and ONEONTA, N.Y. — A.O. Fox Hospital, a not-for-profit community hospital in Oneonta, N.Y. and affiliate of the Bassett Healthcare Network, has deployed Phytel's comprehensive, Web-based population health and patient engagement platform.

The hospital will use the Phytel technology to automate the identification of patients for recommended chronic and preventive care to improve quality and achieve better health outcomes. Phytel is also helping optimize the utilization of the hospital's employed primary care physicians (PCPs) by matching patient needs to clinical expertise and availability.

"Phytel is helping us improve patient health while using our resources more efficiently," said John Remillard, CEO of A.O. Fox Hospital. "I've been a part of this community for a long time. This technology gives us a way to help improve the health of the community by reaching more patients who are in need of recommended care. It will also lay a great foundation for us as we make the transition to more shared risk and value-based care."

The Phytel platform identifies patients with gaps in recommended care, or those who are at high risk of acute health episodes, and engages them through automated and personal follow-up.

Phytel was named the nation's No. 1 population health management solution by the 2014 Best in KLAS Awards: Software Services report. A.O. Fox Hospital selected the Phytel platform in part based on the positive experience and recommendation of clinical and business executives at Bassett Healthcare Network.

Bassett has been using Phytel to help improve patient engagement in their own care since July 2014. A.O. Fox is affiliated with the Bassett Healthcare Network. "We are pleased that Bassett has selected Phytel as a partner in its quality improvement initiatives," said Tom Frosheiser, Chief Operating Officer of Phytel. "We are proud that Bassett Health Network found our approach to be so effective that they recommended it to one of their affiliated hospitals. We look forward to helping A.O. Fox Hospital enhance the already high level of health within its patient community."
