Order Set Solution: Written in compliance with national quality standards to meet key performance measures, this evidence-based library of order sets covers the most frequent hospital and emergency department diagnoses and procedures. The order set templates link to clinically consistent disease, lab and drug information from Thomson Healthcare. The Order Set Solution includes the Thomson Knowledge Manager tool to customize and export order sets, an order item catalog with medical coding, unique Boolean logic operators for appropriate drug selection, and the TALLman lettering best practice recommended by the IOM.

Patient-Specific Dosing: This clinical knowledge tool provides results and recommendations for a specific dose based on renal function, hepatic function, and height and weight of a patient. It improves medication safety by ensuring correct therapeutic dosages are ordered -- preventing drug dosing errors in both adult and pediatric patients as well as high-risk patients with renal or hepatic impairment. Clinicians can access Patient-Specific Dosing through their clinical workflow application and technology systems such as an electronic medical record.

Medication Reconciliation: This Web-based solution enables hospitals to create, maintain and reconcile more accurately a patient's list of medications from admittance to discharge. It significantly reduces the potential for ADEs due to incomplete or incorrect medication lists, and is a specific requirement and a patient safety goal under Joint Commission guidelines. The solution gathers, maintains and reconciles an electronic list of a patient's medications; enables caregivers to continue, discontinue, add or modify medications at each transition point; and features drug identification, interaction checking and e-prescribing tools, and the Patient-Specific Dosing Solution from Thomson Healthcare.
