T-System Helps Hunt Regional Extend Services to Uninsured


Technology will enable hospital to reach patients not eligible for Medicaid or CHIP



DALLAS — T-System, Inc. announced today Greenville, Texas-based nonprofit Hunt Regional Healthcare will implement T-System's PerformNext™ Care Continuity solution to support its effort to better coordinate patient care delivery across a network of physicians, community clinics, quick care facilities and Mental Health Mental Retardation (MHMR) resources.



Section 1115 of the Social Security Act provides federal funding to programs that seek to expand eligibility to patients not otherwise eligible for Medicaid or CHIP - for delivery systems that improve care, increase efficiency and reduce costs. For Hunt Regional, a recipient of Section 1115, Care Continuity will be that delivery system, allowing them to coordinate patient care with healthcare providers across the community.



The web-based application promotes alignment between hospitals and physicians, and allows clinicians to actively managing patient care transitions, with an automated method of sending and receiving patient referrals. It automatically flags patients at highest risk for readmission for follow-up care, reducing the likelihood those patients will return to the ED.



With the Centers for Medicare and Medicaid Services (CMS) Hospital Readmissions Reduction Program in effect, facilities across the country need solutions for managing their patients beyond the moment of discharge.



"We estimate one-third of our current ED patients could be seen by a doctor's office," said Kimberly Mulder, RN, Hunt's Emergency Department Director. "Care Continuity will give us the ability to engage with community resources, and match those patients with appropriate providers so they can get well and stay well."
