Latest Version of Award-Winning Solution Addresses Continuing Demand in Provider Community

CareManager is an award-winning disease management solution which provides physicians the information they need to ensure effective care for their patient populations with chronic disease. By utilizing data stored in the electronic health record (EHR), CareManager allows organizations to improve care quality through timely interventions based on comprehensive clinical information. The 3.3 release includes:

o New filtering capabilities allow organizations to sort and filter patient panels, enabling new levels of patient tracking and identification, including:
- Assessing patient compliance to organizational, as well as national, guidelines
- Effective risk stratification of patients based on multiple risk criteria, ensuring timely outreach to the patients most in need of care
o A new Heart Failure module that allows physicians to manage patients based on health indicators including lowest left ventricle ejection fraction and New York Heart Association score
o A new Osteoporosis module that helps manage at risk patients utilizing bone mineral density test values

CareManager modules are now available for seven disease states, including diabetes, stroke prevention, coronary heart disease, cancer screening, tobacco cessation, heart failure, and osteoporosis. The 3.3 release also includes updates to existing modules, providing the most up-to-date industry established guidelines for the evidence-based care of patients with chronic conditions.

These improvements significantly reduced the risk of heart disease across a diabetic patient population of over 6,000. Providence also documented a 250 percent increase in foot screenings, helping to stem the rate of later amputations. While using CareManager to ensure patients received the care they needed when they needed it, Providence generated more than $4,000 of additional service revenue per physician in the first year of use, without hiring any additional staff.
