Broader Reporting and Trends by Diagnosis: PointRight® Adds New Functionality to PointRight® Pro(TM) 30 Rehospitalization Metric



CAMBRIDGE, Mass. -- PointRight Inc., (www.pointright.com), the industry-standard analytics leader for healthcare providers and payers, announces it has added significant new functionality to its PointRight® Pro 30(TM) rehospitalization service. PointRight has included its Green Light Report capability, which provides a quick, visual scan of facilities that meet or exceed national averages. This enables its users to track facility rehospitalization performance by specific diagnoses and risk groupings. In addition, PointRight has included its Trend Graph capabilities so users can now track these diagnoses and risk groupings over time and against national benchmarks.



Reducing rehospitalization rates is one of the most important quality initiatives for long-term care facilities today, with penalties for excessive rates for acute-care partners reaching as high as 3% in 2014. PointRight Pro 30 is the industry-standard measure that allows operators to easily identify, trend and analyze their rehospitalization cases and perform root-cause analyses to distill quality improvement opportunities, using Web-based, interactive analysis tools.

PointRight Pro 30 helps facilities assess why residents returned to the hospital, determine areas of highest rehospitalization as well as possible preventative measures, and crosswalk rehospitalized resident with other outcomes to improve the quality of care. Using graphical data display tools, it allows clients to benchmark their progress by seeing how rates trend over time compared with national averages. The new capabilities just announced will make it easier for customers to evaluate facility-level rehospitalization rates for common issues including congestive heart failure, COPD, stroke, diabetes, pneumonia and recent surgeries.



PointRight Pro 30 reshospitalization measure has recently surpassed a 9,000 user milestone. It has been adopted by the American Health Care Association (AHCA) and incorporated into its Trend Tracker product, and is in use by a variety of Accountable Care Organizations (ACOs) and Managed Care Organizations (MCOs) nationally, which represents a variety of SNFs in their respective networks. Based upon its success, AHCA has formally submitted the measure to the National Quality Forum (NQF).
