TheTax Relief and Health Care Act of 2006 made permanent the Medicare Recovery Audit Contractor (RAC) program to identify improper Medicare payments. Many hospitals and medical practices have found themselves unprepared for such an audit and the resulting government penalties for discrepancies in documentation accuracy -- both undercoding and overcoding -- between the physician and hospital claims.
