Kryptiq Releases New Version of Its Award-Winning Population Health Management Solution
Healthcare IT company updates mature population health management tool, enabling all care team members to practice at the apex of their licensure.
PORTLAND, Ore. -- Kryptiq Corp. announces the release of CareManager 4.0. Co-developed with leading healthcare organizations, and curated in collaboration with a 42-member customer advisory group, Kryptiq's CareManager aligns the care team with the decisions made at the point of care. Unlike the alternatives, CareManager is deeply integrated within the EHR, is informed by real time clinical data, and provides integrated patient engagement.
Color-coded dashboards present a simple and easily recognizable view into patient risk across the population. Treatment protocols are recommended via CareManager's customer curated guidelines, enabling all providers to practice at the height of their licensure.

According to Cynthia Burghard, Research Director, Accountable Care IT Strategies for IDC Health Insights, this deep integration of care management tools in the provider's EHR system is the primary differentiator between a tool that enables population analytics and a tool that advances care delivery.

CareManager extends the provider's workflow to the patient by generating individualized care plans and patient-friendly, color-coded scorecards. Providers use the scorecard to engage patients in the encounter room, and the opportunity for personal involvement is enhanced because their scorecards are automatically delivered via their patient portal. Additionally the population registry enables quick identification and management of patients that are at risk or symptomatic of a chronic illness, but lack a formal diagnosis.
