Cardiocom® Multi-Disease Management and Heritage Provider Network, Inc. Announce Partnership to Provide Telemonitoring Systems to Community-Based Physicians


MINNEAPOLIS, Aug. 1 / -- Cardiocom® Multi-Disease Management, an industry leading developer and manufacturer of remote patient monitoring equipment and disease management software, announced that it has entered into a relationship with Heritage Provider Network, Inc., a Management Service Organization (HPN, Inc.) extending over Central and Southern California service areas. The two partners will combine the benefits of well-managed Medical Group/IPA networks, which offer value-added clinical resources to community-based physicians, with the innovative and cost-effective technology of an experienced telemonitoring company. The overall goal is to improve the care and clinical outcomes of Heritage Provider Network patients.

The Cardiocom Telescale® and Commander equipment are placed in the homes of HPN Network patients. The sophisticated software system provided by Cardiocom allows nurses at remote locations to monitor daily vital signs and health status of patients. Nurse case managers intervene to provide education and coaching designed to assist HPN patients with the management of their chronic conditions. The telemonitoring system enables physician groups to provide high quality care that follows national practice standards.

Cardiocom and Heritage Provider Network, Inc. have partnered to study best clinical practices resulting from this home based telemonitoring program, as well as the clinical, economic and quality of life benefits that result from patients using the telemonitoring system. Data will be tracked from a minimum of 350 patients with a diagnosis of heart failure. The study will be monitored by HPN clinical research staff including Marilyn Harz, RN, Ph.D, Vice President of Clinical Services, Karen M. Driskill, RN, MHA, Director of Clinical Services and Alan Schechter Ph.D., Director, Healthcare Informatics with Heritage Provider Network, Inc. HPN is also working with Ariel Linden, DPH, MS, a recognized health services researcher whose focus is evaluating the effectiveness of disease management and health promotion initiatives.

The two organizations will enroll patients and deploy the system at six HPN medical groups' sites. Those groups include:

o Desert Medical Group
o Heritage Victor Valley Medical Group
o Greater Covina Medical Group
