A.O. Fox Hospital Selects Population Health Management Platform from Phytel


Population health and care management platform will provide higher quality care, increased collaboration between health providers and a better patient experience



DALLAS and ONEONTA, N.Y. — A.O. Fox Hospital, a not-for-profit community hospital in Oneonta, N.Y. and affiliate of the Bassett Healthcare Network, has deployed Phytel's comprehensive, Web-based population health and patient engagement platform.



The hospital will use the Phytel technology to automate the identification of patients for recommended chronic and preventive care to improve quality and achieve better health outcomes. Phytel is also helping optimize the utilization of the hospital's employed primary care physicians (PCPs) by matching patient needs to clinical expertise and availability.
